Office Policies

New Patients

  • If you are a new patient, please arrive 15 minutes before your scheduled appointment so that paperwork may be updated. Please bring your new patient forms with you.
  • If your insurance (for example, HMO's) requires a referral from your primary care physician, we must have your referral mailed or faxed before your first visit. If our office does not have the referral, your visit may be rescheduled.

At Every Visit, Please Bring:

  • Your insurance card.
  • A complete list of all the prescription medications and over-the-counter medications that you are taking, including doses and frequency.
  • Any recent lab tests or appropriate records that you may have.

Payments and Co-Pays

  • Insurance companies require that co-pays be collected at the time of your visit. If you are paying for your own healthcare, we request that you pay for your office visit on the day of service. Our Patient Accounts Staff is available to meet with you to arrange a payment plan if needed.

Appointment Cancellation

  • If you are unable to keep your appointment, please call the office 24 hours prior to your appointment. There is a $35 charge for missed appointments that have not been cancelled by 24 hours prior, to cover administrative and clinic costs preparing for your visit.
